| 1. Choose the three research programs that most interest you. |

| 2. Complete the
online application when it becomes available in Fall 2008. For part of the application, you will need to prepare a personal statement outlining your interest in the MDITR summer internship program and your relevant background and skills. (Limited to two double spaced pages. ) |

| 3. Obtain a letter of reference: As a courtesy, print out the Reference Form, sign it, and provide your writer with a stamped addressed envelope (see address below). Inform your reference that they should sign their letter and mail it directly to us, not return it to you. In case a Letter of Reference is given to you, you may enclose it when you mail your transcript to us. However, the letter signed by the writer must be submitted in its original sealed envelope, signed across the seal. |

4. Mail your official transcript to:
Acceptance letters for the Hooked on Photonics program will be mailed no later than April 1, 2009. |
